Most laser incidents occur during alignment operations, where the possibility of a misdirected beam is much higher. Administrative controls, safe practices, PPE, and use of the correct alignment aids is vitally important to maintain the safety of the operator(s). However, just simply using the alignment aid is not enough. Knowledge of -how- to use them is critically important and under what conditions their use is appropriate. This talk will present two case studies of incorrect use, and/or, inappropriate conditions of the use of alignment aids that resulted in persistent visual disturbances for the individuals involved.
