Background: Diabetes Mellitus (DM) type 2 has emerged as a global emerging disease in terms of socioeconomic impact and health burden. Other than glycemic control, better outcomes require improved metabolic control. Several studies have suggested that coconut has a positive impact on lipid levels, glucose control, and body weight. However, there were still debates about the metabolic benefits of coconut and its derivatives. As a result, the purpose of this systematic review is to examine the effect of coconut derivatives on metabolic control using interventional trials. Methods: We looked for peer-reviewed studies on the effects of coconut on metabolic control in Pubmed, Scopus, and the Directory of Open Access Journals (DOAJ). This review included a study that ran from 2010 to 2022 on human subjects, randomized control trial using coconut or its derivatives, analyzing lipid level, blood sugar, and insulin resistance; and the full text of the article can be obtained. Three independent reviewers used the Cochrane Risk of Bias Assessment Tool and Grading of Recommendations Assessment, Development, and Evaluation to analyze the evidences. Outcome: There is insufficient evidence to support the clinical use of coconut derivatives to improve diabetic patients’ metabolic control.
