The elderly are a priority target for the COVID-19 vaccination program because they have a higher risk of developing a critical illness and can become fatal. Case Fatality Rate is 11% for those over 60 years old. The elderly who have not accessed the first dose of COVID-19 vaccination are 47.86%. The purpose of this study was to analyze internal and external factors as well as develop strategies to increase the achievement of COVID-19 vaccination in the elderly at the Regional Technical Implementation Unit of the Tampaksiring II Health Center. This study uses a quantitative approach with research subjects conducted by purposive sampling. Collecting data using a questionnaire followed by a Focus Group Discussion. Analysis of the data using the matrix Internal Factors Strategy Analysis and External Factors Strategy Analysis with SWOT analysis. The results of the research on the COVID-19 vaccination program for the elderly at the Regional Technical Implementation Unit of the Tampaksiring II Health Center were in the very good category. The dominant internal factors as strengths are the availability of vaccination guidelines and policies, health human resources, infrastructure, logistics, vaccination service mechanisms and cross-sectoral support. The dominant weakness factor is that integrated information on the achievement of COVID-19 vaccination in the elderly is not yet available in every hamlet of the puskesmas working area. External factors as the biggest threat are physical conditions and comorbidities in the elderly when taking vaccinations, while the dominant opportunities are cross-sectoral support and participation of village officials in helping the elderly access COVID-19 vaccinations. The strategy to increase vaccination achievement is through the Strength Opportunity strategy, by facilitating more accessible vaccination locations such as in the village hall or banjar by involving village officials and cross-sectoral support.
