PT. Nusantara Plastindo is a company that manufactures plastic seeds, raffia rope, and straws. To retain consumer confidence, the company attempts to provide the highest quality plastic seeds and limit the amount of defective goods by requiring plastic sources to account for 2.5 percent of total output. This study attempts to improve the quality of plastic seeds so that fewer faulty products are produced. The failure mode and effect analysis (FMEA) method was used in this study to identify the types and factors that cause defects in the product, which was followed by prioritizing repairs based on the highest RPN value and the Fault Tree Analysis (FTA) method to analyze the root causes of the occurrence. Mode of failure. According to the findings, the most common defective product is aval, which accounts for 51% of all bad products and has four probable failure modes: filthy filters, less homogeneous raw materials, less focused operators, and no preventive maintenance. The FTA technique can look at nine aspects that lead to failure modes in order to come up with nine suggestions for improving plastic pellet production. In general, according to industry requirements, this research can lower the amount of plastic seed faults.
