In this study, bacteria that cause acute and chronic osteomyelitis were isolated and diagnosed in both sexes and all ages. This study was conducted in two hospitals the Surgical Specialist Teaching Hospital, Baghdad Teaching Hospital, and Teaching Laboratories. The study was performed from December 2018 to November 2019. Eighty-five samples were collected the results showed that conducted 80(94.1%) were positive and 5(5.8%) as a negative cultures. The patients were distribution according to age, sex and type of osteomyelitis (acute and chronic). The highest infections were recorded at the ages of (30-39) years 23(28.8%) and the sex which were affected and related with osteomyelitis as males were reported 60(75%) and the chronic osteomyelitis was recorded as a common type of osteomyelitis with rate 50(62.5%). The results of bacterial diagnosis were Staphylococcus aureus 23(21.1%), as a high percentage, Citrobacter spp. and Staphylococcus epidermids 1(0.9%) as a lower percentage. High percentage of resistant of both Gram positive and Gram negative was Cephalexin in Staphylococcus aureus (91.5%) and Pseudomonas spp. (100%). The results of virulence factors was showed that 11(34.3%) of Proteus spp produced heamolysin.. Protease was produced in 8 (26.6%) of Staphylococcus aureus 10 (22.2%) of Escherichia coli was produces -Lactamase while 8(26.6%) in Staphylococcus aureus can produce bacteriocin. The results obtained for adhesion of Staphylococcus aureus revealed that group B was inhibited from (58.3 ± 13.9) to ( 20 ± 10.2) by using ciprofloxacin in sub MIC value of this bacteria.
