Formative assessment instruments combined with the discovery learning model provided feedback on learning activities and improved student learning outcomes during and after the learning process. This research aimed to develop proven formative assessment instruments seen from its validity, reliability, and practicality based on the discovery learning model. This research used the R & D model developed by Borg & Gall consisting of 1) research and information collection, 2) planning, 3) develop preliminary form of products, 4) preliminary field testing, 5) main product revision, 6) main field testing, and 7) operational product revision. The instruments were developed in the form of multiple-choice question, essays, and laboratory performance observation sheet. The results of the validation indicate that the instruments were valid, the trial results showed that the instruments were already reliable, and the results of the user response questionnaire showed that the instruments were practical to use. These formative assessment instruments can be used to measure student’s cognitive and psychomotor learning outcomes.
